Your First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Treatment is usually accomplished the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.

Please provide your dental insurance information when you schedule your appointment. Our staff will verify your coverage and your estimated copay (out of pocket expense) prior to your appointment.
You will be provided with a Username and Password for your online registration.
To expedite your appointment, please visit our Patient Login page and complete your patient registration form using the Username and Password provided.
Please assist us by providing the following information at the time of your appointment:
- The referral card from your Dentist (if you have one)
- Dental Insurance Card
- Photo Identification
- Method of Payment
IMPORTANT: A legal guardian must accompany all patients under 18 years of age.
